Nursing Services
Our registered nurse will come to your home to complete a comprehensive head-to-toe assessment to establish an individualized plan of care for you in collaboration with family members and other multidisciplinary teams involved with your care. You will have an RN case manager that will continue to meet with you and family to reevaluate your plan of care to ensure your needs are being met.
Your nurse will work with you and your doctor throughout your course of treatment.
Personal Care Worker (PCW) Services
A trained personal care worker(s) will be assigned to assist you with your daily living activities and supportive care needs. Providing the following personal and supporting care services:
Bathing
Grooming & Hygiene
Dressing
Toileting
Transferring & Mobility
